The academic suspension appointment request form should only be completed if you have been placed on academic suspension and have a major housed within the College of Applied Arts. The College of Applied Arts Academic Advising Center will contact you after completion to schedule your academic suspension appeal appointment (Contact will be made within 24-48 hours of completion Monday-Friday).
